CMS-408 is not a current Medicare form
There is no current Medicare form CMS-408. Choose CMS-40B to enroll, CMS-L564 for employment information, or CMS-1763 to end coverage through SSA.

At a glance
Correct the form number before filing
CMS-408 is not the current Medicare form for enrolling in or ending Part B. A search for that number may reflect a typo or an outdated secondary reference. Choose the form by what you need Social Security to do.
CMS-40B requests enrollment in Medicare Part B, CMS-L564 supplies employment information for a Special Enrollment Period request, and CMS-1763 requests termination of premium Part A, Part B, or Part B Immunosuppressive Drug Coverage (Part B-ID).
Choose the correct Medicare form
| Your intent | Current form |
|---|---|
| Enroll in Medicare Part B when you already have Part A | CMS-40B, Request for Enrollment in Medicare Part B |
| Document current-employment group health plan coverage for a Special Enrollment Period | CMS-L564, Medicare Request for Employment Information |
| Request to end premium Part A, Part B, or Part B-ID | CMS-1763, Request for Termination of Medicare Coverage |
CMS-L564 accompanies CMS-40B when the current forms require employment information; it does not request enrollment by itself. Use the official links below to open the form for the action you actually need.
Use the official form and submission instructions
Confirm the action you want.
Enrollment, employment verification, and termination use different forms. If you are unsure which action applies, contact Social Security before submitting anything.
Download the form from CMS.
Use the current official PDF and follow the instructions printed on it rather than copying fields from a secondary guide.
Complete the required sections.
CMS-40B has three sections. CMS-L564 assigns Section A to the applicant and Section B to the employer. CMS-1763 asks which eligible coverage you want to end.
Submit the form directly to Social Security.
The current PDFs direct users to mail or fax completed forms to their local Social Security office. Use the SSA office locator for the current destination.
If your intent is to end coverage
CMS-1763 says premium Part A or Part B coverage ends at the close of the month after submission. Part B-ID follows the requested month instead. The form also tells you to contact Social Security before the coverage end date if you change your mind.
Ending coverage can affect access to benefits and future enrollment. Use Social Security or Medicare for advice about your situation rather than relying on a form number found on a third-party page.
Privacy boundary for IRIS
CMS-40B and CMS-1763 require a Medicare Number, and CMS-L564 requests Social Security numbers. Without an applicable written agreement with IRIS, do not submit a form containing a government identifier or highly sensitive medical record through IRIS. Send the correct form directly to Social Security by an accepted method instead.
